Caregiver Counseling – Washington

Caregiver Counseling is a short-term service designed to provide unpaid caregivers additional support in caring for their loved ones. Caregiving is emotionally, mentally and physically demanding. Clarvida recognizes the challenges and joys that come with providing ongoing care to a family member or friend. Caregiver Counseling exists to provide caregivers the support they need to help care for themselves and in turn care for their loved one. Clarvida connects the caregiver with a licensed counselor who will work towards individually tailored goals that best meet the needs of the caregiver. Any further need for support may be facilitated by Clarvida in coordination with the caregiver’s case manager in order to see that their needs are met on an ongoing basis.

Behavior Support Services – Washington

Since 2014, Clarvida has been providing behavioral health care needs to individuals residing in adult family homes and assisted living facilities. These services bring enhanced care and support provided to adults who have been identified as having exceptional care needs due to behavioral concerns.

Our services focus on behavioral support to assist individuals residing in long-term care facilities to maintain their physical and mental well-being and overall quality of life. We do this through direct support and training to the individual as well as with the residential staff, providing them with the tools they need in order to keep individuals health, safety and prevention of decompensation that sometimes corresponds with behavioral health disorders. Behavioral management strategies include both formal and/or informal training and consultative support services for individuals relocating or being diverted from state or local psychiatric hospitals, or who are otherwise at risk for loss of community living options due to challenging behaviors. Services also include psychiatric evaluations, ongoing psychiatric medication management and peer support services.

Health Home Care Coordination (HHCC) – Washington

Health Home Care Coordination (HHCC) is a Medicaid benefit for individuals with higher-than-average health needs. This person-centered, community-based service helps individuals identify, access, and manage various health services, ensuring continuity of care for their physical, emotional, community, and developmental needs.

New Journeys – Washington

New Journeys is an evidence-based program for people 15-40 who are experiencing their first episode of psychosis. The New Journeys program provides comprehensive wrap-around services focused on assisting clients with identifying their personal goals and needs, to allow them to return to functioning well at their job, school, home and other areas of life.
