Qualified Residential Treatment Program (QRTP) Level of Care Assessments – Colorado
Assessments completed by clinical therapist under the supervision of a fully licensed clinician for treatment team consideration regarding appropriate level of care for children/adolescents in multiple settings.
Therapeutic Teen Intervention Program (TTIP) – Colorado
Clinical therapist partnered with a KEEP Teen Case Manager in a DYAD program. Each teen and family is assigned an individual therapist and a KEEP case manager for individual/family therapy and therapeutic case management, psychoeducation and permanency planning.
Key Essential Elements of Permanency (KEEP) Teen Program – Colorado
Clarvida utilizes the KEEP model and specializes in working with adolescents, with the goal of mitigating the need for congregate care.
Foster Care – Colorado
Clarvida has certified foster homes up and down the front-range since 1994. Our foster families have varying specialties to serve children who have diverse needs. We provide in-home support tailored to meet the needs of the child(ren) and families as well as 24/7 on-call support to assist with crisis intervention and incident reporting.
Title XIX IDD Waiver Services – West Virginia
We provide person-centered home and community-based services, including direct services and case management for individuals with developmental disabilities (IDD) and their families. Services include Behavior Support Services, Person Centered Support Services, Nursing Services, Supported Employment Services, and Respite Services.
Child and Adolescent Services – Georgia
Child, Adolescent and Family Services are designed to help children, adolescents and their families with mental health issues. Services may include: Assessment and Recovery Planning, Psychiatric Services, Individual and Family Counseling, School and Community based support, and Intensive Family Intervention.
Youth Support Center – Idaho
Clarvida Youth Support Center admits youth between the ages of 10-17 experiencing a behavioral health crisis, regardless of race, gender, sexual orientation or socioeconomic status. For ages 5-9 we provide resources only. The center operates 24 hours, 7 days a week, 365 days a year. Our center provides a safe, non-judgmental environment, linkage to community resources, and referrals for ongoing care at no cost to those we serve. Individuals may stay at the center up to 23 hours and 59 minutes in one episode of care. During that time, services will include: Medical Screening, Behavioral Health Assessment, Risk Assessment, Solution-Focused Counseling, Case Management, and Peer Support.
Orange County Adult – Wellness Center – Central – California
This is a nationally recognized program that offers an array of groups, classes and resources to increase the wellness of the Orange County community through four key areas Physical, Emotional, Spiritual and Social/Community. The mission of the Wellness Centers (WCs) are “To provide a safe and nurturing environment for each individual to achieve their own vision of recovery while promoting acceptance, dignity and social inclusion”. WCs are entirely peer run programs with each member of the staff having “lived experience” with mental illness, substance use or both and working with their own recovery. Members of the Centers must be 18 years or older, reside in Orange County and are seeking Mental Health Services now or in the past. WCC and WCS each offers 100 groups a week from Arts and Crafts to Yoga, positive thinking, DBSA, AA, Social Hour and a number of Community Integration groups. Bowling, nature hikes, walk about trips to museums, gardens are a few of the excursions offered at the Center.
Orange County Adult – The Peer Mentoring Program – California
The Peer Mentoring Program is a peer run program that assists consumers with mental health conditions to pursue mental health recovery goals. Peer Mentors work alongside consumers to enhance their recovery by focusing on client strengths, modeling recovery practices, and empowering consumers to eliminate the stigma of mental illness.
• Track 1 – Assists Adults and Older Adults with serious and persistent mental illnesses who are referred by county and county-contracted Mental Health Clinics to achieving recovery goals.
• Track 2 – Assists Adults and Older Adults with serious and persistent mental illnesses who are pending discharge from inpatient services at the County Crisis Stabilization Units or regional hospitals to provide linkage to outpatient mental health services within the County of Orange Behavioral Health System
Orange County Adult – CalWORKS – California
CCS-CalWORKs helps participants remove Mental Health and/or substance abuse “barriers” to employment to aid the progress of moving from welfare to active participation in employment activities. CCS-CalWORKs has been providing these much-needed Mental Health and substance abuse services to CalWORKs recipients for the past 16 years. Clinics in Santa Ana and Westminster offer services to Orange County residents at multiple locations. Participants must be California Welfare recipients with Mental Health and/or substance abuse issues and enrolled in CalWORKs or Cal-Learn programs. CCS-CalWORKs provides Evaluation and Assessment Services, Individual and Group Therapy, Crisis Intervention, Case Management, Family Support and Education and Referrals to Community Resources. CCS-CW Evidenced Based Practice (EBP) Services offered: Cognitive Behavioral Therapy, Seeking Safety and Active Parenting.
